Creating Consent Forms

1. To create new consent forms within QuickEMR, you will first need to navigate to Tools --> Patient Portal --> Consent Forms.
2. Press the Create button at the top of the screen.
3. Create a Title for your consent form and enter the body of the consent form within the Consent textbox.
      Note: Use the markdowns to edit the style of the content within your form (ie. # HIPAA Consent - will display as a Header/Title)
4. After reviewing the preview to the right and upon your contentment, press the Save button.

  1. Consent forms can also be designated as a form for Signature of Assignment of Benefits or Signature Release of Information by using the dropdown list under Special Type.  Once these consent forms are signed, the corresponding checkbox and date will be checked and filled in within the Case tab in the signing patients case file.
  2. Marking the Consent Form as "Required" renders its signature a necessity in order for patients to complete the Patient Portal intake form.



How to Send Consent Forms

Once your consent forms have been created within the system, you can then send them to the patient to be signed electronically.  These signed consent forms will then automatically attach to the patients case under the Attachments tab.
1. Navigate to the patients case and select the Attachments tab on the left side of the screen.
2. Select the Consent Forms that you would like to send to the patient (you can select multiple).
3. Press the Invite button.
4. Select your method of either a QR code or email to send the invite to the patient.


The Patients View

1. Upon opening the link or scanning the QR code, the patient will then have the ability to read through the consent forms selected.  They will then check the box for agreement to electronic signature, then sign the consent form.  Once completed they will hit the Submit button.


After the Patient Signs

1. Navigate back to the patients case and select the Attachments tab on the left side of the screen.
2. You will then see all signed consent forms are now under the box "Signed Consent Forms"
3. Click on the signed form to view the consent form with the patient's signature, as well as print, fax, or email if needed.
      Note: the patient's have 1 week to sign the consent forms before the link expires.


Special Type

Consent forms may optionally be given a "special type" that grants them additional behavior. The special types behave as follows:
  1. Signature Assignment - Upon the patient signing and submitting the consent form the signature assignment checkbox and date will be populated on the case record.
  2. Signature Release - Upon the patient signing and submitting the consent form the signature release checkbox and date will be populated on the case record.
  3. Reminder Subscribe - Upon the patient signing and submitting the consent form the first patient phone number on record that is a home or mobile number will be subscribed to reminders in the formats they choose. This gives the patient the option to select text or voice reminders and displays QuickEMRs patient reminder TOS.
  4. Marketing Subscribe - Upon the patient signing and submitting the consent form the first patient phone number on record that is a home or mobile number will be subscribed to marketing in the formats they choose. This gives the patient the option to select text or voice marketing and displays QuickEMRs patient marketing TOS.
